Most people with sciatica have tried resting it, stretching it, or taking anti-inflammatories and gotten temporary relief at best. That's because sciatica isn't a condition in itself. It's a symptom. Something is irritating or compressing the sciatic nerve. Until that underlying cause is identified and addressed, the pain keeps returning. Here's what's actually going on in your body.

The Sciatic Nerve: The Largest Nerve in Your Body

Your sciatic nerve originates from five nerve roots in your lower lumbar spine and sacrum, specifically L4, L5, S1, S2, and S3. These roots merge in the lower pelvis and form a single nerve roughly the diameter of your thumb, which then travels through the buttock, behind the piriformis muscle, down the back of the thigh, and branches below the knee into the tibial and common peroneal nerves that supply your lower leg and foot.

Any irritation, compression, or inflammation at any point along this entire pathway can produce symptoms (pain, numbness, weakness, or tingling) anywhere from your lower back down to your toes. That's why two people can both have "sciatica" and describe completely different pain patterns.

That said, both can exist simultaneously, and early sciatica sometimes feels like deep gluteal or SI joint pain before it travels further down the leg.

Can sciatica go away without treatment?

Mild cases occasionally resolve on their own over several weeks as inflammation settles. However, the underlying mechanical cause (disc position, joint restriction, muscular compression of the nerve) usually remains, which is why sciatica tends to recur. Without addressing the root cause, you're likely managing episodes rather than eliminating them. If your symptoms are more than mild or have persisted beyond 2-3 weeks, an evaluation is worth it.

What makes sciatica worse?
